Snellen Eye Chart
Instructions:
1.) Open the below pdf in a new window by clicking the arrow by its top right corner.
2.) Print out (disable the "Fit to Page" print option), ...or Resize the window to the width of a letter-size paper (8.5 in), or use a dollar bill for scale under the "Big E".
2.) Stand back with your eyes 10 ft (nearly 11 lengths of paper) away from the screen.
3.) Write down the letters of the lowest line that you can read without squinting or straining. Perform separately using each eye and also both eyes together (that's a total of three times).
4.) Record how many of the total number of letters on the line you saw correctly, and also indicating which line (using the labels to the right of the line), which eye(s), and if you used glasses or contact lenses.
